ATHLETIC PARTICIPATION & INSURANCE WAIVER, & MEDIA RELEASE STATEMENT
I acknowledge that there are inherent risks associated and accompanied with sports and activities and that my child may be injured as a result of an accident arising out of participation in athletics or activities.
In consideration for permitting my child named in this forum to participate in sports and/or activities, I release and hold harmless The Baseball College and/or its employees, instructors, coaches, administrators, facilities, et al., from any and all liability including, but not limited to liability for injuries or damages sustained by the individual.
Additionally, player’s personal equipment and possessions are the sole responsibility of the player named above, and The Baseball College is released and will not be held liable for player’s lost or damaged equipment.
For All Players: It is recommended that players wear a protective cup to avoid injuries.
For Catchers: Wearing a protective cup during instruction is mandatory.
Insurance Waiver
I also understand that my child must be covered by medical and/or accident insurance in order to participate in sports and hereby certify that my child is covered for injuries and/or death occurring as a result of participation in baseball instruction with The Baseball College. I also certify that said insurance will be kept in force during the full time that my child engages in the practice or participation in instructional events during the entirety of instructional events at The Baseball College.
Media Release Statement
During the course of instruction, photographs and videos may be taken of players for instructional & promotional use by The Baseball College. I consent to the use of any media by The Baseball College for the intended purposes of instruction, promotion, or newsworthiness.
I (the parent/guardian) have completed all of the information requested in this forum and hereby certify that I have read and agree to all of the statements listed in this forum.
